Grab bag of Microsoft Mangler fixes:

- Support mangling virtual function tables (base tables need work on the
  ManglerContext interface).
- Correct mangling of local scopes (i.e. functions and C++ methods).
- Replace every llvm_unreachable() for actually-reachable code with a
  diagnostic.
