BitcodeReader: Consistently use IsDistinct, NFC 

Consistently use the IsDistinct variable and start relying on it in
GET_OR_DISTINCT.  This change has NFC, but prepares for using IsDistinct
to optimize the behaviour of the getMD() and getMDOrNull() helpers.
