IR: MDNode => Value: Add Instruction::getMDNode()

Add `Instruction::getMDNode()` that casts to `MDNode` before changing
`Instruction::getMetadata()` to return `Value`.  This avoids adding
`cast_or_null<MDNode>` boiler-plate throughout the code.
