Reworked the way Process::RunThreadPlan and the ThreadPlanCallFunction... 

Reworked the way Process::RunThreadPlan and the ThreadPlanCallFunction interoperate to fix problems where
hitting auto-continue signals while running a thread plan would cause us to lose control of the debug 
session.
