Switch Sema::UnusedFileScopedDecls over to a LazyVector.

  - Added LazyVector::erase() to support this use case.
  - Factored out the LazyDecl-of-Decls to RecordData translation in
  the ASTWriter. There is still a pile of code duplication here to
  eliminate.
