llvm-undname: More no-op changes to increase test coverage

- Add test coverage around invalid anon namespaces and
  for error paths in demanglePrimitiveType() and in
  demangleFullyQualifiedTypeName()

- Use DEMANGLE_UNREACHABLE in two more unreachable places
