Clean up Sema::BuildCXXNestedNameSpecifier()'s creation of TypeLoc 

information. Rather than looking at the declaration kind to figure out
what TypeLoc to build, look at the type; it makes so much more
sense. Fixes <rdar://problem/9086649>.
