This allows us to simplify ASTContext::CollectNonClassIvars() by
removing ASTContext::CollectProtocolSynthesizedIvars().  Now all
ivars can be found by either inspecting the ObjCInterfaceDecl and
its companion ObjCImplementationDecl.
