[flang][fir] Add zero_bits operation.

This patch adds the new zero_bits operation and upstrams other changes
including the following:

  - update tablegen syntax to newer forms
  - update memory effects annotations
  - update documentation [NFC]
  - other NFC, such as whitespace and formatting
