[X86][AVX] Fold concat(vpermilps(x,c),vpermilps(y,c)) -> vpermilps(concat(x,y),c) 

Handles PSHUFD/PSHUFLW/PSHUFHW (AVX2) + VPERMILPS (AVX1).

An extra AVX1 PSHUFD->VPERMILPS combine will be added in a future commit.
