DebugInfo: DIRef<> => TypedDebugNodeRef<> 

Delete `DIRef<>`, and replace the remaining uses of it with
`TypedDebugNodeRef<>`.  To minimize code churn, I've added typedefs from
`MDTypeRef` to `DITypeRef` (etc.).
