[spirv] Add spv.GroupNonUniformBallot

This CL also did the following cleanup:
- Moved the test for spv.SubgroupBallotKHR to its own file
- Wrapped generated canonicalization patterns in anonymous namespace
- Updated header comments in SPVOps.td
